What is skilled nursing care?

Skilled nursing care includes Nursing Assessments upon admission and ongoing medication management, IV therapy, wound care, assistance with Activities of Daily Living (bathing, dressing, grooming, toileting, ambulating, etc.), coordination of care needs with the Physician and Therapy, etc.

Who provides skilled nursing care?

Skilled nursing care is provided by registered nurses (RNs), licensed practical nurses (LPNs), certified nursing assistants (CNAs), and other healthcare professionals under the supervision of a physician.

What types of conditions are treated in skilled nursing?

We treat a variety of medical conditions, including post-surgical recovery, chronic illness management, rehabilitation after hospitalization for an injury, stroke, and more.

How can I access skilled nursing services at a CCRC?

Services can be accessed by obtaining a referral from a physician and contacting Pavilion admissions.

Are skilled nursing services covered by insurance?

Many insurance plans, including Medicare and Medicare replacement plans, may cover skilled nursing services for short-stay rehab. Long-term skilled nursing is not typically covered by insurance. It’s important to check with your insurance provider to understand your coverage.

How can family members stay informed about the resident’s care?

Family members are encouraged to communicate with the healthcare team and can participate in care plan meetings.

Yes, visitors are generally allowed, but specific visiting hours and policies may vary.

How is the transition to skilled nursing managed for a new resident?

The transition process involves a comprehensive assessment by our Clinical Liaison, coordination with the resident’s healthcare providers, and an orientation to the new care environment.

What happens if a resident's condition improves?

If a resident's condition improves, the resident may transition back to a lower level of care within Greenwood Village South, such as assisted living or independent living, based on needs and capabilities.
